VLDL cholesterol is rarely measured directly in routine care. Instead, it is estimated from triglycerides on the assumption that VLDL carries most of the triglyceride in the blood. This calculator applies the common rule of triglycerides divided by five and also shows the approximate mmol/L equivalent.
Estimate VLDL cholesterol from triglycerides.
VLDL (mg/dL) = triglycerides ÷ 5 VLDL (mmol/L) ≈ VLDL (mg/dL) ÷ 88.57
The division by five mirrors the VLDL estimate used in the Friedewald LDL equation. It is an approximation, not a direct measurement, and it becomes less reliable when triglycerides are very high. The mmol/L conversion uses the triglyceride factor 88.57 because VLDL cholesterol is estimated from triglyceride mass.
| Symbol | Meaning |
|---|---|
| VLDL | Estimated very-low-density lipoprotein cholesterol. |
| triglycerides | Triglycerides in mg/dL. |
| 5 | The conventional divisor used to estimate VLDL from triglycerides. |
| 88.57 | Divisor used to express the estimate in mmol/L. |
VLDL = 150 ÷ 5 = 30 mg/dL. In mmol/L terms, 30 ÷ 88.57 = 0.34 mmol/L. If triglycerides were 300 mg/dL, the estimate would be 60 mg/dL or about 0.68 mmol/L. The arithmetic is linear, but the underlying assumption weakens as triglycerides climb.
VLDL cholesterol is a derived number in most lipid reports. Laboratories rarely measure it directly because the lipoprotein is heterogeneous and its main cargo is triglyceride. The familiar triglycerides-divided-by-five rule exists to make the Friedewald equation possible, and it inherits the same assumptions. When triglycerides are moderately elevated the estimate is usually reasonable; when they are very high, the relationship between triglyceride mass and VLDL cholesterol becomes less predictable. A high VLDL estimate is therefore best read as a signal to look at triglycerides, HDL and glucose rather than as a stand-alone diagnosis. If your clinician needs a precise lipoprotein profile, a direct measurement or advanced testing may be arranged.

VLDL is a lipoprotein that carries triglycerides from the liver to peripheral tissues. Its cholesterol content is small compared with its triglyceride content, which is why routine labs estimate VLDL cholesterol from triglycerides instead of measuring it directly.
Lipid and metabolic calculations are screening aids, not diagnoses. Friedewald LDL is unreliable when triglycerides are high, and risk depends on blood pressure, smoking, diabetes, kidney disease and family history. Treatment decisions require a clinician and, in most cases, a full lipid panel.
